___ (2)  I will ship (or bring) my bike to your convenient Kona location. Assemble, setup and tune bike and hold for my pickup on or about _____________.  Store my box or case. The charge for this is $60.00.

___ (3)  Disassemble my bike after the race, carefully pack, and hold for my pickup on or about ____________. The charge for this is $50.00.

___ (4)  Disassemble my bike after the race, carefully pack, and ship it to my home address or to the address I have provided below. The charge for this is $165.00.

___ (5)  I will bring my packed bike to your Kona location; please ship it to the address I have provided below. The charge for this is $125.00 if the destination is in the 50 USA states. Other destinations will incur actual shipping charges plus a handling charge.

* Shipping rates are based on an average case or box weight of 42 lbs. If you have a double case or heavy box additional shipping charges will be billed to your account.
